Whether it's a sandboxy hexcrawl or a more scenario-driven series of linked adventures, campaign preparation is, to my mind, one of the central responsibilities of good Game Mastering. There's always room for winging it, but there's simply no substitute for good preparation.

Any Cthulhu session that ends with half the group calmly filling out their last wills and testaments even as their bodies slowly fade into another dimension while another PC is blindly wandering the house looking for soap to "wash the dirt off" while another PC is desperately offering blood sacrifices to a brass head in hopes of finding a solution (and getting eaten by the Entity that lived inside the head) is a successful session in my book. ;)
